Community Health Needs Assessment

As part of the Affordable Care Act, not-for-profit hospitals are required to complete a Community Health Needs Assessment (CHNA) once every three years. This assessment is used by our staff and board to construct a strategic implementation plan to effectively increase the health of our community. The 2019 report includes a comprehensive assessment of the population Blue Mountain Hospital serves, San Juan County, Utah.
